By providing your information in the survey, you consent to its use in accordance with Uber's Privacy Notice found at privacy.uber.com/policy and Tesla’s Customer Privacy Notice at https://tesla.com/legal/privacy, including but not limited to the sharing of your information with Tesla, Uber and/or Qmerit, LLC., and their respective certified installers, partners, agents, and affiliates, for the purposes of determining your eligibility for the promotional offer for a “Free Tesla NACS Wall Connector (MSRP $450) plus up to $500 off installation costs by a certified installer and to facilitate the installation of the equipment. 2. This Promotional Offer is for a limited time only, while supplies last, and is only available for up to 500 Eligible Drivers that meet the Eligibility Requirements herein. The Promotional Offer is in Tesla’s sole discretion and may change at any time.
3. This Promotional Offer is in no way sponsored or administered by Uber Technologies, Inc. or any of their respective partners, affiliates, and subsidiaries. The Promotional offer is a limited time only, while supplies last. This Promotional Offer is subject to change at any time and may be subject to additional terms related to fulfillment of the Promotional Offer.
4. This Promotional Offer is only an invitation to select Drivers. By submitting an Eligibility Survey you are not guaranteed to receive the Promotional Offer. The Promotional Offer is subject to and conditioned upon the Eligible Driver entering into a separate stand-alone purchase transaction (“Purchase Transaction”) with a certified installer selected by the Eligible Driver for the purchase and installation of Tesla’s designated NACS Wall Connector equipment (“Equipment”) , which shall solely govern the purchase and installation of the Equipment. In no event shall Tesla, Qmerit, or any certified installer be required to accept an Eligible Driver’s request to participate in or receive the Promotional Offer. Uber and Tesla disclaims any and all liability or warranties related directly to the installation services of a certified installer under the Purchase Transaction. Eligible Driver shall pay all amounts due under the Purchase Transaction, less any available credits due from the Promotional Offer.

Any questions, comments or complaints regarding the Promotional Offer should be directed to Qmerit and/or the certified installer selected by the Eligible Driver under the Purchase Transaction and Qmerit and/or such certified installer is/are solely responsible for eligibility determinations and the fulfillment of the Promotional Offer. Uber expressly disclaims any and all liability or warranties related to the Promotional Offer, whether express or implied. Uber is not responsible for the promotions, products or services offered in the Promotional Offer, or for the terms and conditions (including financial terms) under which those products and services are offered.
